Admissions Procedures:
Before admittance to the program, the following must be completed

1) A referral from the child’s caseworker from within the Department of Social Services seeking to place the child.
2) Prior approval from the governing CSA (or other pertinent body) authorizing funds for the child’s placement.
3) Complete case history
4) All available current school records
5) All available current health records 
6) A pre-placement screening that will include an initial interview.  It will  give the potential, resident a chance to discuss what their goals and expectations are and to determine if our program is a good fit.
7) Meeting of a team of Back 2 Basics, LLC Staff to decide on acceptance of the potential resident into our program.

A decision will be made within 24 hours after all information has been received and reviewed.  The social worker will then be contacted and informed of the decision.

Within 72 hours of acceptance into our programs  Initial Strategies and Objectives will be developed.  These will be developed with input from the resident, the social worker, staff at the group home and family members who are currently involved in the residents life.
